
To owners and most veterinarians, procedures involving the eyes are scary. Veterinary anesthesia for corneal surgery plays an important role in helping keep patients stable during these delicate procedures. An injury to the cornea can take a turn for the worse quite rapidly, and an ophthalmologist may recommend conjunctival graft surgery in an attempt to save the eye.
Anyone who has spent time in ophthalmology knows these procedures demand an incredible amount of precision. Tiny movements matter. Small changes in intraocular pressure matter. One of the biggest anesthetic goals during surgery is avoiding increases in intraocular pressure (IOP). Coughing, straining, vomiting, or even becoming too light under anesthesia can increase pressure within the eye, which can complicate surgery and potentially affect the outcome of the graft.
Because of this, anesthetic drug choices are made very intentionally. Premedication is usually designed to keep the patient calm and comfortable without causing excessive excitement or nausea. A balanced combination of sedatives and analgesics helps reduce stress before induction begins.
Induction and Airway Management for Ophthalmic Procedures
Induction itself is a particularly important moment in patients requiring eye surgery. The anesthesiologist wants the event to be calm and controlled, with as little coughing or anxiety as possible.
Drugs such as propofol or alfaxalone are commonly used because they allow for a rapid, smooth induction and quick placement of the endotracheal tube.

This is where drugs such as atracurium, cisatracurium or rocuronium become incredibly useful. These drugs induce temporary paralysis of the skeletal muscles, including the muscles controlling eye movement. From the surgeon’s perspective, this provides a beautifully stable surgical field.
From the anesthesiologist’s perspective, however, it raises the level of monitoring required and intra-operative planning significantly.
Mechanical Ventilation and Monitoring During Eye Surgery
A paralyzed patient cannot breathe on their own, so mechanical ventilation becomes mandatory. The anesthesiologist carefully controls the patient’s breathing throughout the procedure, monitoring end-tidal carbon dioxide levels to ensure ventilation remains appropriate.
Oxygenation, heart rate, blood pressure, ECG, temperature, and anesthetic depth are continuously monitored as well.
Neuromuscular Monitoring During Veterinary Anesthesia
Neuromuscular monitoring is another important piece of the puzzle. Peripheral nerve stimulators, such as the StimPod, are used to assess the degree of paralysis and help guide additional dosing if needed.
This allows the anesthesiologist to maintain adequate muscle relaxation for surgery without unnecessarily prolonging recovery.

Recovery is often where the anesthesiologist’s planning really matters. A rough recovery after eye surgery can create sudden increases in intraocular pressure from vocalizing, thrashing, coughing, or straining. That can put stress on fresh sutures and compromise the surgical repair.
Because of this, veterinary anesthesiologists put a tremendous amount of effort into creating calm, quiet recoveries. Pain control is continued into the postoperative period, and some patients benefit from mild sedation during recovery to minimize excitement and dysphoria. Extubation is timed carefully to reduce coughing while still ensuring the patient can protect their airway safely.
Patients that received neuromuscular blocking drugs must also demonstrate complete return of muscle function before recovery progresses too far. The anesthesiologist confirms that the patient is breathing adequately and regaining strength appropriately before considering the paralysis fully reversed.
