
Hemilaminectomy is one of the most commonly performed neurosurgical procedures in small animal practice, particularly for dogs with acute thoracolumbar intervertebral disc herniation (IVDH). Because canine hemilaminectomy anesthesia plays a major role in patient safety and neurologic recovery, numerous factors contribute to the overall prognosis for these patients, including appropriate anesthetic management.
For veterinarians and veterinary technicians, fully understanding the anesthetic considerations and concerns for these patients and the procedure can contribute to improved outcomes. Most everyone in the veterinary community has encountered the “down dog”; the dog who presents with significant neurologic compromise, ranging from ambulatory paraparesis to paraplegia without deep pain. These observations are not just diagnostic, but they directly influence anesthetic priorities.
Why Spinal Cord Perfusion Matters During Hemilaminectomy
Spinal cord perfusion becomes a critical concern, as hypotension and hypoxia may potentiate secondary spinal cord injury. Evidence suggests that anesthesia-associated hypotension can negatively impact neurologic recovery following hemilaminectomy, reinforcing the importance of maintaining adequate mean arterial pressure throughout the procedure. In practical terms, this means proactive blood pressure support using a balanced anesthesia protocol, IV fluid therapy, and vasoactive agents when needed.
Common Anesthetic Complications During Canine Hemilaminectomy
From an intraoperative standpoint, hemilaminectomy is associated with a relatively high incidence of anesthetic complications. A retrospective study of over 200 dogs undergoing thoracolumbar hemilaminectomy found that hypothermia (63.8%), need for mechanical ventilation (63.4%), and hypotension (33.9%) were among the most common complications.
Mechanical ventilation is frequently required due to patient positioning, decreased ventilatory compliance, and anesthetic drug-induced hypoventilation. Instrumentation that includes capnography and arterial blood gas monitoring (when available) are invaluable tools for guiding ventilation and ensuring adequate oxygen delivery.
Multimodal Analgesia for Hemilaminectomy Patients
Analgesia for hemilaminectomy patients should be approached with a multimodal strategy. Constant rate infusions (CRIs), such as fentanyl, ketamine, and/or lidocaine, are commonly used, but regional anesthesia techniques are gaining traction.
The erector spinae plane (ESP) block has been shown to provide effective perioperative analgesia in dogs undergoing hemilaminectomy, with some studies suggesting reduced opioid requirements and improved comfort scores. Incorporating local anesthetic techniques not only improves analgesia but may also reduce inhalant requirements, thereby supporting cardiovascular stability.
Breed-Specific Anesthetic Considerations in Dogs With IVDH
Breed-specific considerations are especially relevant in hemilaminectomy patients, as certain breeds are overrepresented. Dachshunds are the classic IVDH patients, often presenting with acute disc extrusion of varying severities. Their chondrodystrophic conformation predisposes them to disc disease, but from an anesthetic perspective, their small size increases susceptibility to hypothermia and drug dosing variability.
French Bulldogs, another commonly affected breed, present a different set of challenges. As a brachycephalic breed, their characteristic short noses are one of their most endearing attributes, but the less visible airway abnormalities, like an elongated soft palate, hypoplastic trachea, and everted laryngeal saccules, increase the risk of upper airway obstruction, hypoventilation, and difficult recovery.
Preoxygenation, careful airway management, and planning for a potentially prolonged extubation period are critical aspects of the anesthesia plan. Additionally, many French Bulldogs have concurrent airway pathology or subclinical respiratory compromise, which can complicate both ventilation and oxygenation during anesthesia. Their propensity for hypercapnia and hypoxemia further emphasizes the importance of mechanical ventilation and advanced monitoring capabilities.
Positioning and Monitoring During Hemilaminectomy Surgery
When a dog is positioned in the operating room for a hemilaminectomy, they are placed in sternal recumbency. Depending on the surgeon’s preference, a bean bag, towels, or sandbags may be used to stabilize the patient by wrapping them in a “cocoon.”
Given that neurosurgery requires precision and accuracy, a patient who moves even just a little can compromise the surgical approach. Pressure points must be carefully padded to prevent neuropathies and myopathies, particularly in smaller or cachectic patients. Venous return may be compromised, or ventilatory restriction can be induced with overzealous positioning, so a delicate balance must be struck.
Maintaining Normothermia During Canine Hemilaminectomy Anesthesia
Another key anesthetic goal is maintaining normothermia. Hypothermia is not only common but also clinically significant. It contributes to coagulopathy, delayed recovery, and altered drug metabolism.
Active warming devices such as a Bair Hugger, HotDog, or warm water blanket, warmed IV fluids, and even toddler-size socks placed on the paws can help minimize heat loss.
Recovery and Postoperative Management After Hemilaminectomy
Recovery from anesthesia is a particularly vulnerable period for hemilaminectomy patients. These dogs often have limited motor function and may be dysphoric or painful.
Smooth, controlled recoveries are essential to avoid self-trauma or exacerbation of spinal injury. Continued analgesia, sedation when appropriate, and careful handling by trained staff are critical components of postoperative care.
The Role of a Veterinary Anesthesiologist in Complex Neurosurgical Cases
The complexity of these cases underscores the value of involving a board-certified veterinary anesthesiologist (Diplomate of the American College of Veterinary Anesthesia and Analgesia, DACVAA). These specialists bring advanced expertise in managing high-risk patients, tailoring anesthetic protocols, and implementing advanced monitoring and analgesic techniques.
Their involvement can be particularly beneficial in patients with comorbidities, atypical anatomy, or intraoperative instability. In referral and specialty settings, collaboration between the surgeon, anesthesiologist, and nursing team creates a cohesive approach that optimizes patient safety and outcomes.
Careful Anesthetic Planning Supports Better Surgical Outcomes
Anesthesia for canine hemilaminectomy is far from routine. It requires thoughtful planning, precise monitoring, and an understanding of both the surgical procedure and the underlying neurologic disease.
By prioritizing spinal cord perfusion, employing multimodal analgesia, and addressing breed-specific risks, veterinary teams can significantly improve patient outcomes. The addition of a board-certified anesthesiologist further enhances the level of care, ensuring that even the most complex cases are managed with expertise.
